How we can help

1. Family Medical History *

Please note if you or anyone in your household has any medical diagnoses you would like assistance, support, or resources for. If you or anyone in your household has had negative medical experiences you would like assistance with such as obtaining a diagnosis, please note this here as well.

2. Current or Main Concerns for Your Family *

What type of issues are you currently facing? Eviction, debt, foreclosure, food insecurity, transportation problems, overwhelmingness of life, etc.

3. Would you like help with Resource Navigation? *

This means connecting you and helping you apply with the proper local services that will be able to assist you with some of your concerns, that we cannot currently cover at this time.

4. Any goals for your family you want to achieve within the next couple of years? *

This can mean anything. Obtaining housing, building up a savings fund, getting your child into a school, etc.

5. Do you want to join the monthly free book program?

If you choose yes, we will send 2-3 books that are age appropriate for your children every month free of charge until you opt out. This program begins in August 2026.

6. Would you like to participate in future food delivery programs?

We will hopefully be partnering with multiple vendors such as Panera, Whole Foods, Lasagna Love, etc.
